ArrayRx and the ArrayRx Discount Card Program are administered by Moda Health Plan, Inc. (“Moda”). ArrayRx and Moda are collectively referred to in this notice as “we.”

We follow federal civil rights laws. We do not discriminate on the basis of race, religion, color, national origin, age, disability, gender identity, sex or sexual orientation. We do not exclude people or treat them differently because of race, religion, color, national origin, age, disability, gender identity, sex or sexual orientation.

We provide free aides and services to people with disabilities so that they can communicate effectively with us. These include qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, other formats).

If your first language is not English, we will provide free, qualified interpreters and/or information written in other languages.
